At SilverGen GPs, we provide a comprehensive range of general practice services tailored to the needs of aged care residents. Our approach is proactive and preventive. We schedule regular visits and health reviews to stay ahead of problems, rather than only responding when they arise. Our GP services are primarily delivered on-site at residential aged care facilities, supported by telehealth where clinically appropriate or when an in-person visit is not immediately available. Bulk billing for eligible residents.
Regular GP Visits
Scheduled GP visits delivered to your facility by our dedicated team. We focus on building genuine relationships with residents, maintaining continuity of care and ensuring no one falls through the cracks between visits.
Chronic Disease Management
Structured care plans for residents living with diabetes, heart disease, COPD, dementia and other chronic conditions. We coordinate with specialists and allied health to deliver integrated, person-centred care.
Medication Management & Pharmacist Coordination
We work closely with pharmacists and nursing staff to review medications, identify polypharmacy risks and ensure residents are on the safest, most effective treatment regimens.
Preventive Care & Health Assessments
Preventive care, structured health
reviews and risk assessments, including falls, cognition, nutrition, continence and
chronic disease monitoring.
Telehealth Support
When an in-person visit is not immediately available, our GPs provide telehealth consultations to support residents and nursing staff promptly between scheduled visits.
Vaccinations
Administration of influenza, COVID-19, pneumococcal and other recommended vaccinations, coordinated with your facility's infection control protocols.
Palliative Care
Compassionate, dignified support for residents approaching end of life. We work closely with families, nursing staff and palliative specialists to ensure comfort and respectful care.
Minor Procedures
Skin checks, biopsies and minor skin cancer removal performed on-site where clinically appropriate, reducing the need for hospital outpatient appointments and the stress of transport for residents.
Wound Care
Wound assessment and ongoing management delivered in coordination with your nursing team, supporting clinical oversight and reducing avoidable hospital transfers.
Care Coordination
Clear communication, timely referrals, case conferences and family discussions are not an afterthought, they are a core part of how we work at every visit.
